At no additional cost, eligible members may receive:

• $0 copay for visits with a Baylor Scott & White Health cardiologist
• 48 one-way trips to plan-approved locations
• Up to four nutritional counseling visits per year
• Access to the heart failure care program within the MyBSWHealth portal

To participate in the heart health program, you must:

• Have a diagnosis of heart failure
• Receive heart failure care from a Baylor Scott & White Health cardiologist
  • If you do not have a cardiologist or are looking to establish care with a Baylor Scott & White Health cardiologist, we can help you schedule an appointment once you have opted in to the program.
• Be enrolled in one of the eligible BSW SeniorCare Advantage plans for 2027:
  • BSW SeniorCare Advantage HMO-POS Select Rx
  • BSW SeniorCare Advantage HMO-POS Preferred Rx
  • BSW SeniorCare Advantage HMO-POS Premium Rx
• Once you have been accepted into the program, you must log your weight and blood pressure at least twice per month in the heart failure care program within the MyBSWHealth portal.
  • If you do not record your weight and blood pressure for two consecutive months, you’ll no longer be eligible to participate.

Tracking your weight and blood pressure is a simple way to help you and your care team stay on top of your heart failure care. Sudden weight gain can be a sign that your body is holding on to extra fluid, and tracking blood pressure helps show how your heart is responding to your treatment plan. By logging these numbers regularly in the heart failure care program, you can notice changes sooner and share helpful information with your care team. They can support timely adjustments that may help you feel better and avoid worsening symptoms.
